Our Reasonable Fee Schedule for Self-Pay Patients

Initial Psychiatric/Behavioral Evaluation (first appointment):

• 60 minutes consultation – $250
• 90 minutes consultation – $350

Initial Evaluation for Substance Use, Abuse, and Addiction:

• 60 minutes consultation – $350
• 90 minutes consultation – $450

Follow-up Appointments for Medication Evaluation and Treatment Plan Review:

• 10–30 minutes – $100
• 30–45 minutes – $125

We will provide you with a Superbill to submit to your insurance company for out-of-network benefits. However, we do not guarantee reimbursement. It is the patient’s and family’s responsibility to contact their insurance company to confirm reimbursement before seeking our services.

Miscellaneous Fee Schedule

• No-shows for Initial Evaluation – $100
• Cancellations with less than 48 hours’ notice for follow-up visits – $75
• Dictated letters of any nature – $60 per page
• Long-acting injectable antipsychotic monthly shot – $100 per shot, plus session fee
• Long-acting injectable Vivitrol shot – $100 per shot, plus session fee
